Job Summary: A Home Health Physician Orders Support Coordinator (POSC) primarily handles the efficient processing and management of physician orders for home health services, ensuring timely compliance and accuracy. This role involves coordinating with physicians to obtain signed orders, tracking their status, and communicating updates to relevant parties, including intake and Director of Nursing/ DON. A Physician Orders Support Coordinator is also responsible to submit orders via fax or doctor’s alliance platform to PCP’s office, obtaining signed orders for a new and current patient Brockton HHA patients, upload the orders into Axxess and tracking the order’s status. POSC also need to contact the physician’s office to follow up if orders are not signed promptly. The POSC may need to call hospitals, or PCP’s office to request last visits notes or updated medication list after a hospital discharge before the agency can resume services.

Responsibilities

New orders: fax new orders to physicians, upload orders to the patients profile

Tracking Order Status: Maintain records of all orders, their status (pending, signed, etc.), and ensure timely completion.

Communicating with Teams: Inform intake teams and clinicians about the status of orders, including any delays or required actions.

Ensuring Compliance: Verify that all orders are compliant with Medicare and Medicaid also relevant regulations and Brockton Home Health Care agency policies.

Supporting Billing: Coordinate with Clinical managers and DON to ensure accurate and timely claim processing.

Maintaining Records: Keep detailed records of physician orders, including patient demographics, clinical information, and billing details.

Qualifications/Educational Requirements:

  2. Two years of college preferred.
  3. Excellent written and verbal communication skills.
  4. Ability to handle multiple tasks simultaneously and meet deadlines.
